Sex Hormone Binding Globulin (SHGB)
SHBG is a transport protein that binds to sex hormones—mainly testosterone and estrogen—and controls how much is free vs. bound. High SHBG means less free testosterone is available (even if total T is normal). Low SHBG can increase free hormone, but may also signal insulin resistance, liver issues, or androgen excess. It’s a key marker for hormone bioavailability, and crucial for interpreting total vs. free T.
