Cheapest useful panel: total and free testosterone, SHBG and estradiol

A total testosterone result on its own can mislead, which is why the panel below is usually more useful.

Build this panel from individual tests: free and total testosterone, SHBG and estradiol or ultrasensitive estradiol.

A note on LetsGetChecked: the free androgen index is a ratio of total testosterone to SHBG, not a free testosterone measurement. The Endocrine Society recommends measuring free testosterone by equilibrium dialysis or estimating it with an accurate formula (Bhasin et al., J Clin Endocrinol Metab 2018).

Watch the method: the 1,500 ng/dL cap

Not every testosterone test measures the same way. Labcorp OnDemand's total testosterone test says plainly that it "reports total testosterone levels up to 1500 ng/dL. If your level exceeds this, results will show '>1500 ng/dL,' instead of an exact value." Walk-In Lab's Labcorp-based testosterone tests carry the same note.

For a man who isn't on therapy, that rarely matters. A widely used reference range for healthy, non-obese men aged 19 to 39 is 264 to 916 ng/dL (Travison et al., J Clin Endocrinol Metab 2017). On TRT or another hormone protocol, levels can run higher, and a capped result can't show you how high. LC/MS tests like Quest 15983 report the real number. The Endocrine Society has also said direct immunoassays perform poorly at low testosterone levels (Rosner et al., J Clin Endocrinol Metab 2007). More: immunoassay vs LC/MS testosterone tests.

Why total testosterone alone can mislead

Most testosterone in your blood is bound to proteins, mainly SHBG and albumin. Only a small free fraction is unbound. Your SHBG level shifts how much of your total is free, so two men with the same total testosterone can have quite different free testosterone (Goldman et al., Endocr Rev 2017).

This matters in practice. In a study of 3,334 men aged 40 to 79, men with normal total testosterone but low calculated free testosterone reported more sexual and physical symptoms than men with both normal. They also tended to be older and have higher SHBG. Men with low total testosterone but normal free testosterone didn't show those symptoms (Antonio et al., J Clin Endocrinol Metab 2016).

That's why the Endocrine Society recommends adding free testosterone when total testosterone is near the lower limit, or when a condition alters SHBG. It also recommends a fasting morning draw, repeated on a second day, before low testosterone is diagnosed (Bhasin et al. 2018). Where the others are the better choice

  • Labcorp OnDemand is the fastest. Results usually come 1 day after your sample reaches the lab. Ours typically take about 3 days for a single hormone test.
  • At-home kits skip the lab trip. Everlywell and LetsGetChecked mail you a finger-prick kit. We don't offer at-home collection. Finger-prick results may not match a venous draw for every marker; see can at-home finger-prick kits replace a lab draw?
  • Quest direct comes to you. It offers in-home collection for $79 more, where available.
  • Walk-In Lab lets you pick your lab. Choose Quest or Labcorp, and order from a much larger catalog.
  • Marek offers bigger ready-made panels and specialty tests we don't sell.
  • Your doctor and insurance may cost you less. If your plan covers the test, that can beat any cash price here.

FAQs

What is the cheapest way to get a testosterone test without insurance?

The lowest price we found on October 11, 2026, was $14 for an LC/MS total testosterone test at Quest through Anabolic Insights, with no extra fees. Marek Diagnostics was next at $42, including its draw fee. Lab-direct and at-home options ranged from $51.75 on sale to $89.

How much does a testosterone test cost at Quest or Labcorp without insurance?

Bought directly from Quest, its Testosterone Test is $69 plus a $6 physician service fee. Labcorp OnDemand's Total Testosterone Test lists at $69 and was $51.75 on an October sale. The same Quest LC/MS test is $14 through Anabolic Insights.

Is a free testosterone test worth adding?

Often, yes. SHBG changes how much of your total testosterone is free, and men with normal total but low free testosterone can still have symptoms. At Quest through Anabolic Insights, free and total testosterone together cost $18.

Are at-home finger-prick testosterone tests accurate?

They can be useful for a first look, but neither Everlywell nor LetsGetChecked publishes the testing method on its testosterone product page. A venous draw at a lab offers more test options, including LC/MS and free testosterone by dialysis. For tracking over time, use the same method and lab each time.

Do I need a doctor's order for a cash-pay testosterone test?

Not one from your own doctor. Every service here uses an independent or partner provider to authorize the order. At Anabolic Insights, licensed providers order the lab panels, and the results are educational, not a diagnosis.

Why does my testosterone result say greater than 1500?

Some immunoassays stop reporting at 1,500 ng/dL, so your real level is somewhere above that. If you need the exact number, retest with an uncapped LC/MS method.
